Option forms

This section contains the various option forms of the Fund. As all these option forms require your signature you need to print out the option form and submit it direct to the Fund.

The following option forms are available:

Retirement Form – Living Annuity Option from the CPUT Retirement Fund

Dependents and nominees form

Investment Switch Form
